Part I |
List your musical accomplishments. Include information regarding your major area of emphasis
(e.g., trombone, violin, soprano), any solo or ensemble work you have done, music groups in
which you have actively participated, music honors you have received, and any other pertinent
information regarding your music career to this point.

Part II |
List any other honors or awards you have received (both in music and in other areas).

Part III |
Briefly describe your community service and volunteer efforts accomplished in your high school years.

Part IV |
Write a brief essay (not to exceed 500 words) describing your career goals in music. Include information
on the college or university you intend to attend, the major you will pursue, and the job you expect to
seek upon the completion of your degree. Tell why you wish to become a music educator.

Part V |
Include two letters of reference with your application. One of the references should be from one of
your current music teachers. Both letters should affirm your music abilities and your potential as a
music educator.
